To use our streamlined private-prescription pathway, your doctor should complete
the short registration PDF and return it directly to LSDC Pharmacy from their
official work email address.

Registration normally only needs to be completed once. The form asks for basic professional
details so that our pharmacy team can verify the prescriber.

What the Patient Needs to Do

Follow these simple steps to help your doctor complete the registration process.

Send the PDF to your doctor or the doctor's medical secretary.

Ask the doctor to complete it.

The doctor must email the completed PDF directly to pharmacy@lsdc.uk from their official work email address.

LSDC Pharmacy will review the details and contact the doctor if anything further is required.

Dear Doctor/Secretary, LSDC Pharmacy has asked my prescribing doctor to complete its short registration form before future private prescriptions can be processed through its verified-prescriber pathway. Please complete the attached PDF, then email it directly from the doctor's official work email address to pharmacy@lsdc.uk. Registration normally only needs to be completed once. Thank you.

Please download and complete the registration PDF, then email the completed file from your official hospital, clinic, practice, university or organisational email address to pharmacy@lsdc.uk.

Theform requests only the basic professional details required for verification:

  • Full registered name
  • Professional title
  • Specialty
  • GMC number or other professional registration details
  • Official work email address
  • Practice or organisation
  • Practice telephone number and address
  • Optional medical secretary details
  • A short professional declaration
